Job summary

Hanker Systems, Inc. is seeking an experienced LVN to provide hospice and skilled nursing care under RN/MD supervision. You will perform routine patient visits, administer medications, execute clinical procedures, and manage wound care.

You will educate patients and families, implement care plans, and coordinate with the interdisciplinary care team. On-call duties may be required based on agency needs, including weekend shifts.

Qualifications

• Active LVN license in the state of practice.
• BLS certification required.
• Recent hospice experience required.
• Epic experience required.
• Experience with medication administration and clinical procedures.
• Strong patient-care and communication skills.
• Must be available to work weekends and 3 weekdays.
