The role requires at least 1 year of LVN experience with chemical dependency or co-occurring disorders, current CA LVN license, and BLS. Strong documentation, teamwork, and patient-centered care are essential.

Qualifications

  • Current BLS certification and active California LVN status.
  • Ability to work independently under medical director supervision.

Responsibilities

  • Complete beneficiary health screenings and medication reconciliations at intake.
  • Audit medication logs, oversee self-administration, coordinate refills.
  • Participate in multidisciplinary team meetings and case conferences.
  • Provide medical case management including primary care, obstetric, mental health, and MAT appointments.
  • Upload medical documentation to EHR and notify medical director for review.
  • Complete withdrawal management scales for detox clients and report changes.
  • Triage beneficiaries’ medical needs with the medical director.
  • Oversee TB testing for clients and staff.

Position DescriptionThe licensed vocational nurse is responsible for providing direct client assessment, medication oversight, and medical case management services to beneficiaries in a residential substance use treatment setting under the auspices of the medical director. He/she works collaboratively with the treatment team to provide education and support to beneficiaries regarding their medications and to coordinate referrals to outside medical services.Primary ResponsibilitiesComplete beneficiary health screening questionnaire and medication reconciliation forms during the beneficiaries’ intake process. Medical data should be collected and recorded in a comprehensive, accurate and systematic mannerComplete pill counts, audit medication log, oversee client self-administration of medications, coordinate medication refills, and resolve beneficiary issues with prescriptionsParticipate in multidisciplinary team meetings and case conferencesProvide medical case management services to include primary care, obstetric, mental health, and MAT appointmentsReceive medical documentation from clients, upload to electronic health record system, and notify medical director for reviewComplete withdrawal management scales for clients in detoxification program, document changes and report to medical director as necessaryWork with medical director to triage beneficiaries’ medical needs and daily medical statusOversee TB testing for clients and staffPerform additional duties as assigned by supervisor and/or medical directorQualificationsMinimum 1-year experience as a licensed vocational nurse with experience treating clients with chemical dependency, mental health and/or co-occurring disordersMaintains current BLS certification and active California LVN statusMust be able to work independently under the auspices of the medical directorMust discharge duties within the licensure and scope of a licensed vocational nurseAbility to effectively and efficiently perform all job duties utilizing clear written and verbal communication in a collaborative team environment.Exhibits excellent organizational and time management skills with the ability to prioritize workload in a fast paced, intensive, client centered environmentAbility to work effectively with people differing in value systems, ethnicity, cultural backgrounds, language capabilities and disabilitiesUtilize tact and diplomacy; exercise good judgment and respond calmly in emergency situationsMust obtain CPR/First Aid certification, annual TB test and complete annual required trainingsMust pass comprehensive background check, reference check, and possess a valid driver license, current auto liability insurance and a clean driving recordPhysical RequirementsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
