The role includes medication administration, wound and IV care as allowed, and ongoing resident and family education.

Qualifications

  • LVN graduate from an accredited School of Nursing.
  • Current, valid LVN licensure in the state.

Job Details: Location: Bremerton, WA 98310; Salary Range: $40.00 - $50.00 hourly.

Position Summary

The LVN implements the established plan of care for each assigned group of residents. Responsibilities include total care for chronically ill and technologically dependent residents, administration of medications, performance of treatments, provision of resident/family education, and record maintenance. Under the direct supervision of the RN, assists with planning, coordination, and provision of individualized resident care in accordance with the facility’s policies.

Essential Job Functions
  • Perform assigned resident care duties to meet physical, psychosocial, and spiritual needs of chronically ill and technologically dependent residents, incorporating cultural and ethnic factors.
  • Complete initial and ongoing assessments, gathering data timely, considering functional/developmental age factors.
  • Differentiate between normal and abnormal clinical findings; intervene per clinical standards and physician orders.
  • Collaborate with resident/family and interdisciplinary team to develop individualized care plans.
  • Use clinical judgment in implementing and evaluating nursing aspects of interdisciplinary care plans.
  • Monitor and report resident condition changes to RN, physician, family, team, and Director of Nursing or ADON.
  • Administer medications proficiently, including pain management.
  • Provide IV care if certified, within scope and facility policy.
  • Administer treatments per physician directions, including wound packing, irrigation, catheter care, etc.
  • Manage technologically dependent residents: ventilator, ostomies, feeding tubes, tracheostomy care, suctioning.
  • Identify resident/family educational needs; provide individualized teaching; document effectiveness.
  • Understand age‑specific care: safety, normal values, skin integrity, behaviors; communicate clearly.
  • Monitor and document signs/symptoms of abuse/neglect; report per policy and regulation.

Physical Capacities
  • Move intermittently; bend, squat, twist, reach continuously.

  • Stand and walk extended periods.
  • Lift and carry up to 25 pounds; lift up to 100 pounds with assistance infrequently.
  • Push and pull carts; transport residents.
  • Cope with mental and emotional stress.
Environmental Conditions

Inside the facility: normal temperatures, some noise, occasional fumes or odors, chemical exposure, potential exposure to bloodborne pathogens. The role requires working 24‑hour skilled care with exposure to emergency responses, holidays, unusual resident behavior, family reactions, death and dying, state surveys, and interactions with medical staff.

Qualifications & Requirements
Education
  • LVN graduate from an accredited School of Nursing.
License & Certification
  • Current, valid LVN licensure in the state.
  • Current BCLS certification.
Work Experience
  • Minimum 6 months LVN experience in a general acute care hospital within past 2 years, or equivalent competency for sub‑acute residents.
Language Skills
  • Read, analyze, and interpret common scientific and technical information; communicate verbally in English.
Mathematical Skills
  • Add, subtract, multiply, divide using whole numbers, fractions, decimals in units of measure.
Communication Skills
  • Exceptional communication and customer service skills; empathetic.
