Job summary

Bond Health seeks an LVN to provide medical care and supervision for adolescent clients in a residential facility in Texas. The nurse reports to the Program Director and ensures nursing services meet licensure and policy requirements.

Responsibilities include medication administration, assessments, referrals, staff training in BLS/CPR, and documentation across all required records. A valid Texas LVN license and driving privileges are required.

Qualifications

  • LVN license by the state of Texas.
  • Preferred nursing experience, with at least one year in mental health or pediatric nursing.
  • Must maintain a valid driver's license, insurance, and safe driving record.

Responsibilities

  • Adheres to the Nurse Practice Act while providing medical care to clients.
  • Implements assessments and arranges referrals for outsourced medical resources.
  • Administers medication and supervises self-administration with accurate documentation.
  • Monitors medication supplies and ensures documentation is completed and audited.
  • Provides client education on health topics to maintain licensure.
  • Participates in program events and staff meetings; maintains compliance with policy.
  • Performs nursing assessments and related evaluations per procedures and accreditations.
  • Assists in psychiatric evaluation scheduling and patient follow-up.
  • Provides utilization review of required documentation per standards.
  • Administers staff TB testing and trains non-medical staff in BLS/ CPR, AED, first aid, Narcan.

LVN'S needed at a residential facility
POSITION SUMMARY:

Reporting to the Program Director, the Nurse is responsible for medical care and supervision of adolescent clients in a residential facility.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Adheres to the laws and rules outlined in The Nurse Practice Act under The Texas State Board of Nursing; while providing medical care to clients.
  • Implements accurate Assessments and arranges referrals for adolescent clients needing outsourced medical resources.
  • Administers medication and medical treatment as per policy to include supervising staff and clients in self-administration of medication, ensuring accurate and appropriate documentation is completed and providing training for staff as needed.
  • Monitors and audit medication supplies, both prescribed and over the counter medication as per policy.
  • Supervises and ensures all documentation is completed, filed, audited, and submitted as required.
  • Provide client education on health and wellness topics as per required to maintain licensure.
  • Monitors and participates in program events and activities.
  • Performs nursing assessments and other required evaluations as per Policy and Procedures; in addition to the standards expected and maintaining specified accreditations.
  • Assist in psychiatric evaluation scheduling, follow up appointment, documentation, and triage organization; regarding patient follow up according to change in patient status.
  • Provides utilization review of required documentation in accordance with the expectations, as per professional code and state licensing standards.
  • Administers staff TB testing in compliance with Policy and Procedure as needed.
  • Provide training on BLS/ CPR, AED, first aid, Narcan, and other required topics to non-medical staff.
  • Attends all required staff training sessions in accordance with and state licensing requirements, completes all training on time
  • Attends and participates in program and all staff meetings
  • Adheres to and promotes the Mission, Vision, and Values while acting as a role model for others
  • Other duties as assigned
EDUCATION/EXPERIENCE/CREDENTIALS:
  • LVN license by the state of Texas
  • Preferred Nursing experience, with at least one-year experience in mental health or pediatric nursing
  • Must maintain a valid Drivers' License, have proof of automobile insurance and maintain a safe driving record.
