Visiting Wellness is a home-based behavioral health provider seeking Licensed Mental Health Counselors (LMHCs) and Licensed Clinical Social Workers (LCSWs) to serve pediatric and adolescent patients in Brooklyn, NY. The role requires a Master’s degree in a behavioral health field and an active NY licensure, with the ability to work in patients' homes

Qualifications

Responsibilities

Summary

Visiting Wellness, a provider of home-based behavioral health services, is seeking Licensed Mental Health Counselors (LMHCs) and Licensed Clinical Social Workers (LCSWs) to provide high-quality, evidence-based mental health services to pediatric and adolescent patients in Brooklyn, NY 11208.

As a Licensed Therapist, you will conduct comprehensive mental health assessments, develop individualized treatment plans, and provide individual, family, and caregiver counseling using evidence-based therapeutic approaches.

Responsibilities

• Conduct comprehensive mental health assessments, including psychosocial evaluations and diagnostic assessments.
• Develop individualized treatment plans tailored to each patient's clinical needs and goals.
• Provide individual, family, and caregiver counseling using evidence-based therapeutic approaches, including CBT, Solution-Focused Therapy, Mindfulness-Based Interventions, Trauma-Informed Care, and other evidence-based treatment modalities.
• Monitor patient progress and adjust treatment plans as appropriate.
• Collaborate with physicians, rehabilitation professionals, social workers, and other members of the interdisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation, including evaluations, treatment plans, and progress notes.
• Educate patients and families regarding treatment goals and coping strategies.
• Participate in clinical meetings, supervision, and continuing education opportunities.
• Maintain compliance with all federal, state, agency, and ethical standards.

Qualifications

• Master's degree or higher in Mental Health Counseling, Social Work, Psychology, or a related behavioral health field.
• Active New York State LMHC or LCSW license in good standing.
• Experience providing therapy services to children and adolescents preferred.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently in patients' homes while maintaining professional standards.
