Job summary

Family Health Centers of San Diego is seeking licensed mental health professionals for outpatient counseling in a hybrid San Diego, CA role. The position requires active CA licensure (LMFT/LCSW/LPCC/Clinical Psychologist) and focuses on delivering counseling sessions, documentation, and collaborative care planning with a multidisciplinary team.

You will perform initial assessments, develop individualized care plans, and coordinate with case managers while connecting patients to resources.

Qualifications

• Active California license in LMFT, LCSW, LPCC or Clinical Psychologist.
• Outpatient mental health experience preferred.
• Strong documentation and coordination skills required.

Responsibilities

• Provide counseling sessions to assigned patients.
• Complete all required documentation accurately and timely.
• Perform initial assessment and ongoing care.
• Develop individualized care plans.
• Address co-occurring disorders.
• Work as a member of a multi-disciplinary treatment team.
• Coordinate with Case Managers and patient support staff.
• Connect patients to internal/external resources as appropriate.
• Perform other duties as assigned.
