Job summary

Family Health Centers of San Diego (FHCSD) is seeking a Licensed Therapist to join our outpatient team in a hybrid position. The role focuses on counseling, assessments, care planning, and coordinated care within a multidisciplinary team.

CA licensure as LMFT, LCSW, LPCC or Clinical Psychologist is required. The hospital offers a starting salary of $100,000–$125,000, a $5,000 signing bonus, relocation assistance, and a comprehensive benefits package.

Qualifications

• Must have active California licensure as LMFT, LCSW, LPCC or Clinical Psychologist.
• Experience providing counseling to outpatient populations and coordinating care with multidisciplinary teams.

Responsibilities

• Provide counseling sessions to assigned patients
• Complete all required documentation accurately and timely
• Perform initial assessment and ongoing care
• Develop individualized care plans
• Address co-occurring disorders
• Work as a member of the multi-disciplinary treatment team
• Coordinate with Case Managers and patient support staff
• Connect patients to internal/external resources as appropriate
• Performs other duties as assigned
