Job summary

Transitions, Inc. in Erlanger, KY, is seeking a Licensed Substance Use Disorder Therapist for our Residential program. You will conduct assessments, develop treatment plans, and deliver individual and group therapy as part of a multidisciplinary team.

Qualifications

• Master’s degree in Behavioral Health or related field.
• Active Kentucky license (CSW, LCSW, LPCA, LPCC, LPA) and/or CADC.

Responsibilities

• Manage a caseload of clients in residential and outpatient settings, including assessments, treatment plans, individual and group therapy.
• Develop and update individualized treatment plans ensuring documentation meets regulatory and clinical standards.
• Participate in weekly team meetings and coordinate care with staff and external referral sources.
• Engage in weekly individual supervision with a Licensed Clinical Supervisor.
• Create discharge plans including safe housing arrangements and referrals to lower levels of care.
• Provide crisis intervention and coordinate with other treatment providers as needed.
• Represent Transitions professionally in all interactions with clients, families, and referral sources.
