Job summary
Focus Healthcare of Tennessee LLC in Tennessee seeks an experienced addiction/mental health counselor to provide intensive counseling services to a caseload of patients. Qualifications
- Master's degree in psychology, social work, counseling, sociology, or equivalent.
- At least two years of individual and/or group counseling experience.
- Licensed in TN as a Professional Counselor, Clinical Social Worker, MFT, MSW, or in process.
- Recovering individuals must have two years of abstinence and active support-group participation.
Responsibilities
- Assess assigned patients and develop individualized treatment plans.
- Conduct individual and family counseling sessions with patients.
- Document sessions in EMR within established timeframes.
- Conduct group therapy sessions as scheduled.
- Facilitate psychoeducational groups as assigned and scheduled.
- Support discharge planning and coordination of referrals.
