Qualifications

• Must have a bachelor’s or master’s degree in Social Work.
• Experience in a skilled nursing or sub-acute health care setting is preferred.
• Must possess a current valid Maryland Social Work license.

Responsibilities

• Coordinates medically related social services for residents.
• Assists in meeting psychosocial needs of residents.
• Addresses advance directives and medical decision making.
