Job summary

Avir at Hearne is seeking a Licensed Social Worker to assess psycho-social needs, develop care plans, and support residents’ well-being in collaboration with the interdisciplinary care team.

You will conduct psychosocial assessments, provide individual and group therapy, facilitate family sessions, monitor care plan effectiveness, and maintain accurate records to ensure patient‑centered, high‑quality care in a fast-paced setting.

Qualifications

• Licensed Social Worker (LMSW or LCSW) in Texas.
• Bachelor's degree in Social Work or related field.
• Strong interpersonal and communication skills.
• Ability to work in a fast-paced environment.
• Passion for delivering exceptional patient-centered care.

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans to address residents' physical, emotional, and social needs.
• Provide individual and group therapy sessions to support residents' mental health and well-being.
• Facilitate family support sessions and other social activities to enhance quality of life for residents.
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
• Maintain accurate and timely records of resident interactions and assessments.
