Job summary

LTC Therapy Services is seeking a Licensed Social Worker (MSW) to join our leadership team as a full-time Social Services Manager. You will oversee the Social Services Department while guiding residents and families through admissions, care planning, and discharge transitions.

You will collaborate with nursing, rehabilitation, and administration to ensure compassionate, person-centered care while maintaining compliance with state and federal regulations.

Qualifications

• Master's degree in Social Work or related field.
• Active Social Work license (LICSW/LCSW/LSW) required.
• Experience in social work, admissions and discharge planning in long-term care.

Responsibilities

• Lead the Social Services Department and advocate for residents' psychosocial well-being.
• Coordinate admissions, discharge planning, and interdisciplinary care planning.
• Complete social services assessments, including BIMS, PHQ-9, trauma screenings, and other required documentation.
• Communicate with residents, families, physicians, and the interdisciplinary team to ensure continuity of care.
• Maintain accurate documentation in PointClickCare.
• Support regulatory compliance and survey readiness activities.

We're seeking a Licensed Social Worker (MSW required) to join our leadership team as a full-time Social Services Manager. In this role, you'll oversee the Social Services Department while helping residents and families navigate admissions, care planning, and successful discharge transitions.

As an important member of our interdisciplinary team, you'll collaborate with nursing, rehabilitation, and administration to ensure residents receive compassionate, person-centered care while maintaining compliance with state and federal regulations.

Responsibilities include:

• Lead the Social Services Department and advocate for residents\' psychosocial well-being.
• Coordinate admissions, discharge planning, and interdisciplinary care planning.
