AdventHealth in Calhoun, GA seeks a part-time Social Worker to support patients and families in understanding and following medical plans. You will develop Plans of Care, connect with community resources, and coordinate with the care team to ensure smooth discharge planning.

Qualifications include a Master’s in Social Work, 1+ year in community health (preferred), and a valid driver’s license. CPR/BLS certification preferred. This role offers meaningful work and community impact.

Qualifications

• Master's in social work from an approved program.
• 1+ year experience in community health (preferred).
• 1+ year experience in Medicare-certified home health (preferred).
• Must be fluent in English.
• Driver's license required (in licenses section).

Responsabilités

• Assist patients and caregivers in understanding, accepting, and following medical recommendations.
• Develop individualized Plans of Care to address patients' needs and adjustments.
• Refer patients and families to appropriate community resources.
• Communicate with the primary nurse regarding patient needs and concerns and participate in case conferences and discharge planning.
• Document patient interactions and case information accurately.
• Serve as a resource for nursing staff regarding available community assistance.
• Provide advocacy and liaison services to facilitate necessary interventions.
• Conduct in-service education and participate in staff meetings.
• Engage in public relations activities and maintain community interfaces with agencies and other professionals.
• Provide consultative services regarding social service needs.
• Initiate patient contact and conduct home visits to assess social, emotional, and financial factors.

Job Description

Offers advocacy and liaison services with individuals or agencies to facilitate necessary interventions. Conducts in-service education and participates in staff meetings to promote professional development. Engages in public relations activities and maintains community interfaces with agencies and other professionals. Provides consultative services regarding social service needs. Other duties as assigned.Initiates patient contact and conducts home visits to assess social, emotional, and financial factors.

Knowledge, Skills, and Abilities

• Must be fluent in the English language. [Required]
• Valid Florida Driver’s License and safe driving record. [Required]

Education

• Master's [Required]

Field of Study

• Graduate of an approved social work program.

Work Experience

• 1+ experience in community health [Preferred]
• 1+ experience in medicare certified home health agency [Preferred]
