Job summary

The Social Worker – MSW provides clinical social work services to patients and families to support functioning, adjustment, and decision-making across the healthcare continuum. Qualifications

• MSW from CSWE-accredited program
• 2–4 years clinical social work experience in healthcare preferred

Responsibilities

• Perform psychosocial assessments across age groups to evaluate needs.
• Provide counseling and resource education to patients and families.
• Coordinate discharge planning and post-acute care services.
• Refer to internal and external mental health and social services.
• Manage high-risk referrals and complex psychosocial cases.
• Document in medical records per standards.
• Support care planning and safe discharge with the team.
