Job summary

Psycho Social Therapies in Ohio is seeking a Licensed Social Worker or Licensed Professional Counselor for part-time work in geriatric care facilities. You will provide individualized talk therapy, assessments, and care planning to residents in nursing homes and assisted living facilities.

You will collaborate with nursing staff, case managers, and other caregivers; monitor progress; maintain confidential records; and help clients cope with anxiety, depression, and other mental health

Qualifications

• Current valid state licensure as LSW/LISW or LPC.
• Experience providing talk therapy in geriatric settings preferred.
• Ability to collaborate with nursing staff and case managers.
• Strong record-keeping and confidentiality practices.

Responsibilities

• Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
• Develop customized care plans for each client, considering their unique circumstances and mental health requirements.
• Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively.
• Regularly assess clients’ progress and adjust care plans as needed.
• Document treatments, interventions, and services provided.
• Engage in therapeutic conversations with clients to address emotional and behavioral issues.

Licensed Social Worker or Licensed Prefessional Counselor (Part-Time)

Psycho Social Therapies is a leading mental health agency specializing in long-term care, servicing the geriatric population residing in nursing homes and assisted living facilities throughout the state of Ohio. Our experienced and dedicated team assist the residents transition into a long-term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues. Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and/or assisted living facility, ensuring that each client's unique mental health needs are met with compassion and expertise.

Responsibilities

1. Evaluating Clients:

• Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
• Identify individualized support options based on these evaluations.

2. Creating Individualized Care Plans:

• Develop customized care plans for each client, considering their unique circumstances and mental health requirements.

3. Collaborating with Other Care Providers:

• Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively.
• Facilitate seamless communication and holistic care for clients.

4. Monitoring Clients:

• Regularly assess clients’ progress and adjust care plans as needed.
• Keep track of treatment outcomes and make necessary modifications.

5. Maintaining Records:

• Document treatments, interventions, and services provided.
• Ensure accurate and confidential record-keeping.

6. Providing Talk Therapy:
