Psychosocial Therapies in Ohio is seeking a Licensed Social Worker (LSW/LISW) or Licensed Professional Counselor (LPC/LPCC) to support geriatric residents in nursing homes and assisted living facilities. The role focuses on assessments, care planning, coordination with clinical teams, and delivering talk therapy to address anxiety, depression, and other mental health needs.

• Ensure accurate and confidential record-keeping.

Psycho Social Therapies is a leading mental health agency specializing in long-term care, servicing the geriatric population residing in nursing homes and assisted living facilities throughout the state of Ohio. Our experienced and dedicated team assist the residents transition into a long-term care facility and continue to work with them on any challenges they may be facing, such as anxiety, depression, and other mental health issues. Our Licensed Social Workers and Counselors deliver personalized services within the nursing home and/or assisted living facility, ensuring that each client's unique mental health needs are met with compassion and expertise.

Responsibilities:

1. Evaluating Clients

• Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
• Identify individualized support options based on these evaluations.

2. Creating Individualized Care Plans

• Develop customized care plans for each client, considering their unique circumstances and mental health requirements.

3. Collaborating with Other Care Providers
