The role involves assessing needs, developing care plans, coordinating with the care team, admissions and discharges, and maintaining confidential records. OH LSW license and strong communication are essential.

Qualifications

• Bachelor’s degree in Social Work (preferred).
• Active Licensed Social Worker (LSW) in Ohio.
• Previous experience working with older adults preferred.
• Strong interpersonal and communication skills.

Responsibilities

• Provide emotional support and guidance to residents and families.
