You will perform MDS and PHQ-9 assessments, coordinate discharge planning, and develop comprehensive care plans with physicians, nursing staff, therapists, and families.

This role emphasizes advocacy, education, and best-practice implementation to improve resident outcomes and quality of life.

Qualifications

• Current LSW license in the State of Ohio.
• Knowledge of social work principles, practices, and applicable regulations.
• Experience with MDS assessments and PHQ-9 documentation.
• Strong communication, organization, and interpersonal skills.

Responsibilities

• Complete and oversee MDS and PHQ-9 assessments, ensuring accuracy and regulatory compliance.
• Coordinate and manage discharge planning, helping residents transition safely and successfully to their next level of care.
• Conduct social service and recreational assessments to support resident well-being and quality of life.
• Collaborate with physicians, nursing staff, therapists, residents, and families to develop comprehensive care plans.
• Provide recommendations that support positive resident outcomes and enhance care delivery.
• Assist the facility in identifying and implementing best practices related to resident care and social services.
• Maintain accurate documentation in accordance with federal, state, and facility guidelines.
• Support residents and families through advocacy, education, and resource coordination.
• Perform additional duties as assigned.

What You’ll Do
