Job summary

Fundamental is seeking a Licensed Social Worker (LSW) to lead the Social Services Department in a full-time role alongside discharge planners and family members to coordinate tours and follow-up procedures. The position involves overseeing planning, development, and evaluation to meet federal, state, and local standards.

The role requires a current Social Work license in good standing, a Bachelor's in Social Work preferred, and at least 3+ years in a long-term care facility.

Qualifications

• Must have a current Social Work license in good standing with the state (as required).
• Bachelor’s degree in Social Work strongly preferred.
• 3+ years of experience in a long term care facility.
• Must be able to organize and prioritize many tasks effectively.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing the Social Services Department.
• Develop and manage initial contact with discharge planner/family to initiate tour and follow up procedures.
• Identify medically-related social and emotional needs of patients/residents.
• Maintain good working rapport with intra-department personnel and external social agencies.
• Stay current with federal and state regulations and professional standards.
