Job summary

Ohio Department of Behavioral Health in Cincinnati seeks a licensed independent social worker (LISW) to provide clinical treatment planning and delivery within a unionized, hourly role. Qualifications

• Requires LISW license issued by Ohio Board of Counselor, Social Worker & Marriage & Family Therapist.
• Official transcripts required for all post-secondary education listed on the application.

Responsibilities

• Develop, implement, and regularly review individualized treatment plans with patients and interdisciplinary team.
• Monitor patient progress and modify interventions based on clinical needs and treatment response.
• Document clinical services assessments, interventions, and patient progress accurately and timely.
• Identify patient strengths, treatment needs, risk factors, barriers to discharge and available support.
• Collaborate with legal representatives, courts, probation/parole personnel and other authorized agencies as appropriate.
• Assist with evaluations and treatment activities related to competency restoration, criminal responsibility, or other forensic treatment requirements.
• Maintain awareness of court orders, legal requirements and patient rights affecting treatment and discharge planning.
• Develop and sustain collaborative relationships with external facilities, agencies, and support services to facilitate referrals and ensure effective follow up.
• Assess housing, transportation, financial, employment, healthcare and community support needs.
• Coordinate referrals to outpatient mental health services, residential programs, community providers and other resources.
