Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient needs and strengths.

• Support patients during acute mental health crises such as suicidal ideation.

• Develop care plans integrating social determinants of health with behavioral health.

• Coordinate care plans with the care team and document progress notes.
