Job summary

Highland Pines is seeking a Licensed Baccalaureate Social Worker (LBSW) in Texas for a full-time role in a long-term care facility. The position emphasizes psychosocial assessment, care planning, and coordination with an interdisciplinary team to support residents' emotional well-being.

The ideal candidate holds a TX LBSW license, a BSW or related degree, and at least one year of supervised social work in healthcare with geriatric patients. Prior nursing facility experience is preferred.

Qualifications

• Certified, licensed, or registered in Texas is required.
• Bachelor’s Degree in Social Work or related field required.
• Minimum of 1 year of supervised social work in a healthcare setting with geriatric residents.
• Nursing facility experience preferred.

Responsibilities

• Complete social history and psychosocial assessments for residents.
• Maintain compliant documentation across required records and care plans.
• Support residents’ self-determination and educate family members as needed.
• Develop interdisciplinary care plans addressing psychosocial needs.
• Coordinate discharge planning and referrals for home care services.
• Promote resident rights and monitor psychosocial well-being.
