Job summary

Outfield Healthcare Partners is seeking a licensed Social Worker to foster resident well-being and independence in a skilled nursing setting. You will assess psychosocial needs, document thoroughly, and support residents’ self-determination in collaboration with the care team.

Qualifications

• Licensed social worker (state registration/certification) required.
• At least 1 year of supervised social work experience in a healthcare setting with geriatric residents.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Complete social histories and psychosocial assessments for each resident.
• Maintain accurate documentation per federal/state regulations and center policies.
• Support resident self-determination and educate on therapeutic options.
• Participate in developing an interdisciplinary plan of care addressing psychosocial needs.
