Job summary

Morningstar Residential Care Center is seeking a Social Service Leader to oversee social services in a skilled nursing facility. You will orient and educate staff, develop programs and policies, and conduct case work and counseling to support patient discharge planning.

You will participate in performance improvement activities and CQI initiatives, ensuring compliant, patient-centered care within the facility.

Qualifications

• Bachelor’s degree in Social Work is required.
• Current Clinical Social Work licensure in New York State.
• Minimum of three years in social services and discharge planning.

Responsibilities

• Provide consultation and direction of Social Service in the facility.
• Orient and educate Social Service and Nursing staff.
• Develop, organize, and implement programs, policies and procedures.
• Conduct case work services and counseling.
• Recommend functions to ensure operational viability.
• Maintain performance improvement activities and participate in CQI.
