Qualifications

• Licensed Social Worker with required licensure per state regulations.
• Experience in performing evaluations and services across age groups.
• Strong data collection and documentation skills.

Responsibilities

• Performs vocational rehabilitation assessment and dictates interdisciplinary report.
• Initiates and discharges documents within required time frames.
• Documents therapy notes in the clinical record.
• Develops and reviews initial and comprehensive treatment plans.
• Identifies and documents patient care needs.
• Collaborates with interdisciplinary team to ensure patient care and rights.
• Provides patient/family emotional support and education.
• Meets in-service requirements including mandatory education.
• Performs other duties as required per Physician Services policies.
