Job Summary: Provides comprehensive psychosocial services, assessment, diagnosis, evidence-based treatment, case management, and triage for patients, families, and caregivers in an ambulatory outpatient setting.

Essential Functions
  1. Provides biopsychosocial assessments, clinical behavioral health diagnoses, treatment planning, evidence-based clinical interventions, and referrals to community resources.
  2. Interviews patients and their social support systems to complete initial psychosocial evaluations or assessments.
  3. Provides brief psychotherapy, supportive counseling, and education to patients and their social support systems, helping them cope with stress related to hospitalization, disability, or chronic/terminal illness.
  4. Develops, monitors, and revises care plans based on assessments, in collaboration with a multidisciplinary team.
  5. Provides and evaluates the effectiveness of patient/family education.
  6. Advocates for patients and families throughout the care continuum, serving as a resource to facilitate communication among providers.
  7. Provides clinical supervision to provisional licensed social workers and mentors new staff.
  8. Serves as a resource regarding emotional, social, and psychosocial aspects of patient illnesses.
  9. Intervenes effectively in crisis situations using crisis intervention skills.
  10. Participates in developing clinical pathways, protocols, and population-based programs, fostering team collaboration.
  11. Analyzes and evaluates the impact of social work on quality outcomes and system performance, implementing improvement strategies.
  12. Refers patients and families to external agencies/resources when appropriate.
  13. Documents assessments, notes, education, and outcomes accurately and timely according to departmental policies.
  14. Participates in clinical research and may present at local, national, or international levels.
Physical Requirements

Must have adequate hearing, vision, and speech for communication. Ability to push wheelchairs, lift up to 50 pounds occasionally, and transfer patients up to 100 pounds rarely. Requires sitting, standing, walking, and occasionally climbing stairs or reaching overhead.

Education, Experience, and Certifications
  • Master’s degree in Social Work from a CSWE-accredited program.
  • Minimum 1 year of experience within the department or 2 years healthcare experience for external candidates.
  • Licensed Clinical Social Worker (LCSW) in NC and/or SC.
  • Membership in a professional organization required; additional certifications preferred.
  • Ability to mentor and supervise new social workers as needed.
