Overview

Advocate for patients at one of the most pivotal points in their care. Gateway Regional Medical Center is looking for a Licensed Social Worker to join our Case Management team. You'll work independently to review medical records, assess medical necessity, and help guide appropriate hospital stays and ancillary services—all in alignment with Joint Commission and Medicare standards. It's a role that blends clinical judgment with genuine advocacy for patients navigating their care.

What You'll Do

• Provide social services in accordance with TJC and Medicare standards.
• Conduct daily review of medical records to assess appropriateness and medical necessity of admission, continued stay, and ancillary services.
• Function independently within the direction set by Case Management.
• Support patients and families through care transitions and discharge planning.

What You Bring

• MSW from a CSWE‑accredited program, or a BSW from a CSWE‑accredited institution.
• Illinois LSW licensure (required within 6 months of hire).
• LCSW or LMSW preferred.
• Prior healthcare experience preferred.
