Job summary

The Meadows of Central Massachusetts, part of Vibra Healthcare, seeks a Social Worker to provide patient assessment, counseling, and discharge planning. You will work as part of an inter-disciplinary team with physicians, nurses and case management to support patients and families.

Minimum qualifications include a Bachelor’s Degree in Social Work and an active license in the state; Master’s Degree in Social Work is preferred and two years of healthcare experience is desirable.

Qualifications

• Bachelor’s Degree in Social Work is required and a current, valid license to practice as a Social Worker in the state.
• Master of Social Work is preferred but not required.
• Two years of experience as a Social Worker in a healthcare setting is preferred.
• Knowledge of regulatory standards and compliance requirements is important.
• Excellent organizational, prioritizing and analytical skills are essential.
• Ability to make independent decisions when circumstances warrant.
• Working knowledge of computer and software applications used in job functions.

Responsibilities

• Provide patient assessment, counseling, and discharge planning.
• Act as liaison between patient/family/hospital and community agencies/resources.
• Collaborate as part of an inter-disciplinary team with physicians, nurses and case management.
• Apply psychosocial theory to assist patients and families in improving social functioning.
• Occasionally plan educational offerings and provide in‑services as requested.
