Qualifications

• Master’s degree in social work is required.
• Licensed Clinical Social Worker or Licensed Social Worker in Hawaii is required.
• Basic Life Support certification from the American Heart Association is required.
• Preferred: minimum one year of social work experience, healthcare experience with medical diagnoses, and strong community networking skills.

Responsibilities

• Conduct psychosocial assessments and develop and evaluate social work care plans; provide therapy for individuals, couples, families, and groups.
• Perform case management, resource referral, advocacy, mediation, and ensure appropriate patient placement.
• Contribute to program development, implementation, evaluation and quality improvement initiatives.
• Engage in research, documentation, and regulatory compliance.
