Job summary

Continuum Care Hospice is seeking a licensed social worker (MSW) to join our interdisciplinary hospice team in the Cleveland market. The role focuses on planning, organizing, and directing social services for patients and families in accordance with regulations and policies.

Qualifications

• MSW from an accredited School of Social Work.
• Active LSW license in good standing.
• Minimum of two years of supervised social work experience in a healthcare setting.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Knowledge of psychosocial principles, community resources, and healthcare systems.

Responsibilities

• Conduct comprehensive psychosocial assessments of patients and families.
• Develop and implement individualized plans of care addressing psychosocial, emotional, and environmental needs.
• Provide counseling, emotional support, crisis intervention, and bereavement support to patients and families.
• Assist patients and families with community resources, financial assistance programs, advance directives, and healthcare decision-making.
• Collaborate with physicians, nurses, chaplains, aides, and other interdisciplinary team members to ensure coordinated care.
• Participate in care planning meetings and maintain accurate clinical documentation.
• Advocate for patient rights and ensure compliance with hospice regulations and standards.
• Assist with discharge planning and transitions of care when appropriate.
• Maintain confidentiality and professionalism in all patient interactions.

Licensed Social Worker (Hospice) – Continuum Care Hospice

Position Summary

Continuum Care Hospice is seeking a compassionate, dedicated, and experiencedLicensed Social Worker (MSW) in the Cleveland market to join our interdisciplinary hospice team. The Licensed Social Worker will assist in planning, organizing, implementing, evaluating, and directing the overall operation of social services for hospice patients and their families in accordance with current federal, state, and local regulations, as well as Continuum Care Hospice policies and procedures.

The ideal candidate is committed to providing patient-centered care, emotional support, advocacy, and resource coordination to individuals facing life-limiting illnesses and their loved ones.

Education and Experience

Required Qualifications

• Minimum oftwo (2) years of supervised social work experience in a healthcare setting working directly with individuals.
• Knowledge of psychosocial principles, community resources, and healthcare systems.

• Ability to work independently and collaboratively within an interdisciplinary team.

Preferred Qualifications

• Previous hospice, palliative care, home health, or long-term care experience.
• Knowledge of end-of-life care practices and bereavement support services.
