Hopkins Restorative Care Center is seeking a licensed social worker (LSW) to support residents and families from admission through discharge. You’ll coordinate services with the care team and help create smooth transitions.

A Minnesota Social Work license is required, and prior skilled nursing or long-term care experience is preferred. This is a visible, hands-on role that values accountability and collaborative problem-solving in a fast-paced environment.

Formación

• Licensed Social Worker (LSW) required.
• Experience in skilled nursing or long-term care preferred.
• Strong communication and relationship-building skills.
• Ability to coordinate care across departments and with families.

Responsabilidades

• Support residents and families from admission through discharge.
• Coordinate services with nursing, therapy, and facility leadership to ensure smooth transitions.
• Assist with care planning and barrier removal to improve the resident experience.

Hopkins Restorative Care Center is looking for a Licensed Social Worker to join our team and play a key role in supporting residents and families from admission through discharge.

With our newly refreshed TCU, you’ll work closely with short-term rehab residents, families, nursing, therapy, and facility leadership to coordinate services, remove barriers, support care planning, and help create smooth, successful transitions.
