Job summary

Hallettsville Nursing and Rehabilitation Center seeks a licensed social worker to oversee admissions, assess resident needs, and coordinate referrals to community resources. You will help ensure social and mental health well-being, develop relationships with residents and families, and support the care team during the admission process.

Qualifications

• Bachelor’s or Master’s in Social Work.
• Must be a Licensed Social Worker in the state of Texas.
• Experience in a skilled nursing facility preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Oversee admissions process and identify resident needs.
• Make referrals to community resources and coordinate social services.
• Ensure residents' mental and social health is supported; counsel residents and families.
• Complete psychosocial assessments and contribute to care planning and MDS when needed.
• Participate in care plan meetings, QA reporting, and department updates.
• Assist with resident/family concern management and routine investigations when required.

Primary ResponsibilitiesThe Social Worker will be responsible to work with the residents in overseeing and assisting in the admission process, identification of needs and problems, make referrals to community resources, and ensuring the mental and social health of each resident.Essential FunctionsDevelop relationships with residents and their responsible party as needed for admissions, counseling, and identifying needsWork with responsible party and resident on social interaction, reality orientation, and intellectual stimulationAssist Business Office Manager in solving financial needs and promote emotional security for the residentComplete acute care plans as needed and attend care plan meetingsAssist Administrator with resident/family/responsible party concern managementAttend committee meetings as required.Complete monthly QA reports.As needed, refer residents for vision, dental, and hearing servicesAssist with room changes by contacting responsible party and resident and completing necessary formsComplete the initial, routine, and as needed psychosocial and socialization assessment, if needed the LSW can refer resident for additional professional psychosocial services.Discuss advance care planning with responsible party upon admission and as per state regulations and complete necessary documentsMake morning rounds prior to morning meetingBe prepared to provide staff with any important department updates at the department head daily morning meeting Perform suicidal/homicidal assessments and required follow up Assist in investigations as requested by AdministratorAssist with the completion of the MDS as neededWork rotating weekend Manager on duty as scheduledEducational/Training RequirementsBachelor’s or Master’s in Social WorkLicensing RequirementsMust be a Licensed Social Worker in the state of TexasExperience RequirementsPrevious experience working in a skilled nursing facility is preferredPhysical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
