Licensed Social Worker - Job Description:
On the Center for Substance Recovery, the LSW works in collaboration with the patient, nurses, physicians and interdisciplinary team. This role provides comprehensive substance use disorder assessments (ASAM), complex discharge planning and provides therapy for patients while on the unit. The social worker assists patients and families to identify the psychosocial barriers that impact their recovery and discharge planning process. The Social Worker provides assessment, advocacy, education about the effects of substance use, along with the process of recovery, referral and linkage to the next appropriate level of care for continued substance use services, coordination, counselling, and crisis intervention to assigned patients. Services are provided in the inpatient settings. This role works is in conjunction with the multi-disciplinary team to develop a discharge plan that is safe and effective for the patient. The role is responsible for the implementation and coordination of the transition of care needs under the direction of the System Director of Behavioral Health and Center for Substance Recovery.
