Job summary

Responsibilities

• Conduct comprehensive psychosocial assessments to evaluate client needs and develop individualized care plans.
• Provide individual, group, and family counseling and therapeutic interventions.
• Coordinate care and connect clients with community resources, support services, and referrals.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Collaborate with interdisciplinary teams including physicians, case managers, and behavioral health professionals.
• Advocate for client rights and support clients through crisis intervention when needed.
• Monitor client progress and adjust treatment plans to ensure optimal outcomes.
