Job summary

Ledgewood Rehabilitation & Skilled Nursing Center in Beverly, MA is seeking a compassionate Licensed Social Worker for a 16-hour-per-week part-time role. You will support residents and families through every stage of their journey, contributing to person-centered care in a warm, collaborative environment.

Ideal candidates hold a B.A. in Social Work with current licensure; MSW with LCSW/LICSW is preferred. At least 1 year of SNF experience is preferred to join our dedicated team.

Responsibilities

• Compiles social history data.
• Participates in assessments and care plan development.
• Leads weekly interdisciplinary care planning meetings.
• Documents progress notes per state and federal regulations.
• Coordinates discharge planning for residents.
• Interprets and applies social service policies and regulations.
• Coordinates services with community agencies upon discharge.

Requirements for Licensed Social Worker

• Must possess a B.A in Social Work, LSW or equivalent degree AND a current social work license.
• Masters in Social Work (MSW), and LCSW or LICSW higher preferred.
• 1+ years skilled nursing facility experience preferred

Job Duties of Licensed Social Worker

• Compiles social history data.
• Participates in assessments, development and evolution of resident's care plan.
• Responsible for planning and leadership interdisciplinary care planning weekly meetings.
• Documents timely progress notes according to state and federal regulations.
• Coordinates the discharge planning for residents.
• Able to interpret and implement established social service policies, procedures and regulations.
• Coordinates services of community agencies for continued delivery of care upon discharge.
