Intersect-Healthcare-3 is seeking a Social Service Manager to provide medically related social work services, assessing psychosocial needs and coordinating with an interdisciplinary team to support residents’ well-being.

The role emphasizes discharge planning, family support, community resource referrals, and supervision of social work interns, with a focus on resident rights and care planning within the facility.

Qualifications

• Requires a bachelor’s degree in social work and at least one year experience in a social service program for the elderly or related field.
• Must be licensed (full or limited) as a bachelor’s level clinical social worker with the State of Michigan.

Responsibilities

• Assess and evaluate residents' psychosocial needs and set goals for providing the necessary service and participate in admission when needed.
• Integrate social service goals into the care plan and attend care planning conferences.
• Assist residents and families to utilize community resources when not provided directly by the corporation.
• Maintain confidential records and interview residents and families as appropriate.
• Provide supervision for social work student interns.
• Assist in development, supervision and education of staff.
• Lead discharge planning as team lead.
• Serve on the marketing committee as a member.
• Coordinate services with psychiatric providers and OBRA requirements.
• Assist the Clinical IDT in resident room management and advocate for resident rights.
