Job summary

Diversicare of Foley seeks a qualified Social Worker to support admissions, care planning and family counseling for residents in our post-acute settings. You will work with an interdisciplinary team to coordinate discharge planning and advocate for residents’ rights.

Qualifications

• Bachelor’s or Master’s Degree in Social Work.
• License with State.
• One year of supervised social work experience in a health care setting.
• Experience in healthcare settings – nursing home, hospice, home health, or hospital preferred.
• Ability to work with diverse people in varied settings, from community to nursing home planning.
• Ability to make decisions based on evaluations and case histories.

Responsibilities

• Assist in pre-admission and admission process with customers; aid clients in adjusting to nursing home placement.
• Interview residents and families to obtain past and present adjustments, behavior and lifestyle.
• Document progress and update plans of care at the interdisciplinary care conference.
• Arrange transportation, schedule appointments, and liaise between departments, residents’ families and outside agencies.
• Assist in handling and accepting placement through counseling and group work.
• Develop discharge plans and post-discharge services.
• Assess residents’ emotional, financial, spiritual, educational and social needs; ensure needs are met.
• Advocate for residents and explain their rights.
• Organize and conduct Family Counsel Meetings and keep minutes.
• Maintain effective communication with residents, families, healthcare professionals and staff.
