Job summary

4C Health is seeking a licensed outpatient therapist to provide assessments and therapeutic services for individuals, couples, families and groups at 32 hours per week in Indiana.

Under supervision, you will participate in treatment planning with center staff and maintain detailed documentation. Four-day work week, with additional professional development and tuition assistance available.

Qualifications

• Master’s Degree in Behavioral Health or related field required.
• Licensed credentials: LCSW/LCAC/LMFT/LMHC required or eligible.
• Experience with mental health, addictions and behavioral health practices preferred for direct staff roles.

Responsibilities

• Provides assessments and therapeutic services to individuals, couples, families, and groups.
• Provides mental health, addictions and behavioral health clinical treatment within scope of practice.
• Participates in treatment planning with the center psychiatrist or HSPP and client.
• Reports client progress to appropriate referral source and maintains documentation/reports.
• Supervision for licensure and ongoing professional development is provided.

This position provides assessments and therapeutic services to individuals, couples, families, and groups. This position works to provide mental health, addictions and behavioral health clinical treatment/diagnosis within scope of practice as defined by degree and license. This position is responsible for participating in treatment planning with the center psychiatrist and client, reporting client progress to appropriate referral source, and maintain adequate and timely documentation. This position operates under minimal supervision and must exercise tact, judgment and confidentiality in contact with the public, clients and families, as well as fellow associates of the Center.
