Tender Touch Rehab Services is seeking a licensed Physical Therapist Assistant for a full-time role in a SNF/long-term care setting. You will work under a licensed Physical Therapist, helping identify patients for therapy and coordinate evaluations and referrals.

Qualifications

• Associate degree from an APTA-accredited PTA program.
• Current PTA license in the state of practice.

Responsibilities

• Identify appropriate patients for therapy and coordinate with the PT for evaluation.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure physician orders are obtained and up to date.
