Job summary

Lehigh Valley Health Network in the Lehigh Valley seeks a licensed mental health professional to provide assessment, diagnosis, treatment, and crisis intervention for adults and/or children with a broad range of needs.

You will oversee utilization management, discharge planning, clinical assessments, case management, and care coordination; act as a liaison between patients, providers, and funders; and participate in quality improvement activities.

Qualifications

  • Master's Degree Social Work, Counseling, or related field.
  • Ability to remain patient and calm in stressful situations.
  • Must have knowledge of community resources and understanding of how to make appropriate referrals to community and other governmental agencies for services.
  • Understanding of the psychosocial/medical needs of clients in order to formulate an integrated, comprehensive treatment plan.
  • LCSW - Licensed Clinical Social Worker - State of Pennsylvania
  • LPC - Licensed Professional Counselor - State of Pennsylvania
  • LSW - Licensed Social Worker - State of Pennsylvania

Responsibilities

  • Oversees all utilization management activity to include authorization of services as well as management of denial and appeal process.
  • Ensures all patient discharge planning documentation is clear, concise, timely, and congruent with treatment team recommendations.
  • Conducts clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.
  • Provides case management and care coordination services for clients/patients.
  • Intercedes on behalf of the individual patients to assist them in achieving and maintaining community stability.
  • Acts as a liaison between patients, referral sources, and funders.
  • Manages all elements of mental health - voluntary and involuntary commitment processes as regulated by Pennsylvania Mental Health Procedures Act. (chapter 5100 regs)
  • Participates in quality improvement activities for the department and or program.

Summary

Physical Demands

Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
