Responsibilities

• Completes needs assessments based on clinical input from providers.
• Develops plan for intervention/care and conducts group therapy as deemed appropriate.
• Performs telephone triage for patients in crisis and documents contacts.
• Uses EPIC EMR for documentation of patient visits and correspondence.
