Job description

Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

We are seeking a Therapeutic Behavioral Specialist to join our team! You will provide recovery-oriented support and case management services to residents of long-term care facilities and their families. Opportunities may exist for future transition to providing counseling services. We care for individuals in long-term care settings, addressing issues from anxiety and depression to severe mental illness and end-of-life concerns. As part of a multidisciplinary team, our clinicians significantly impact residents' quality of life by solving problems, delivering clinical care, and serving as a critical link in their care.

Essential Duties and Responsibilities:

1. Provide Therapeutic Behavioral Services by consulting with a licensed clinician to meet individual needs.
2. Develop service plans for supports or care coordination, including healthcare and behavioral health treatment plans.
3. Implement linkage, interventions, treatment options, daily skills restoration, and crisis prevention services.
4. Deliver services to individual or group clients.
5. Train support staff on client triggers, de-escalation techniques, and interventions as per treatment plans.
6. Foster positive relationships with outside contacts.
7. Coordinate with Resident Care Coordinator or staff upon facility visits to review care needs and communicate findings.
8. Maintain open communication with facility staff and management, providing feedback on treatment plans and caregiver information.
9. Adhere to Eventus WholeHealth standards, policies, procedures, legal, and regulatory guidelines, including HIPAA compliance.
10. Conduct oneself professionally at all times and maintain patient confidentiality.
11. Consult with the clinical supervisor on unfamiliar resident, family, or facility requests related to care.

Qualifications/Education:

Master’s degree in counseling or marriage and family therapy, with eligibility for state licensure as a Licensed Professional Counselor or Licensed Marriage and Family Therapist.

Knowledge:

• Population-specific interventions and care following treatment plans.
• Rules and regulations governing behavioral health practice.
• Organizational policies and procedures.

Skills Required:

• Data gathering and analysis of physiological, socioeconomic, behavioral, and emotional patient information.
• Training in social/emotional well-being, stress management, anger management, and life skills.
• Accurate evaluation of patient problems and appropriate intervention or referral.
