Job summary

Harmony Senior Services in Murfreesboro, TN, seeks an LPN to coordinate medication management, assess resident needs, and ensure accurate documentation. You will work with a team to support wellness, safety, and nutrition, while delivering compassionate care in a fast-paced environment.

This role requires an active LPN license, experience with seniors, supervisory ability, strong communication and time management, plus flexibility to work weekends and holidays.

Qualifications

• Current LPN license to practice nursing in the state where the community is located.
• Experience with the senior population, preferably in assisted living or LTC.
• Ability to supervise staff and work well with others.
• Strong verbal and written communication skills.
• Effective time management and organizational abilities.
• Must be able to lift up to 50 lbs and maintain physical condition.
• Willing to work an 8-16 hour flexible schedule including weekends/holidays.
• Attend required trainings, CEUs and staff meetings.
• Must be willing to take/state certification classes as required.

Responsibilities

• Execute the medication management plan with periodic review and physician communication.
• Continuously assess resident needs with updates to care and services.
• Implement measures to ensure resident wellness and safety with proper documentation.
• Work in a team to meet resident needs.
• Respond to emergencies timely and calmly.
• Report incidents and changes in resident condition with appropriate documentation.
• Participate in activities including community events.
• Assist with meals in the dining room and monitor nutrition.
• Maintain high standards for record keeping and documentation.
• Establish positive working relationships with all departments.
• Identify risk factors and initiate interventions with documentation.
• Provide thorough reports of resident status during shifts and in the Daily Log.
• Mentor and direct CNAs/RMAs.
• Attend trainings, CEUs and staff meetings.
• Adhere to policies, safety procedures, and body mechanics.
• Complete all other assigned duties.

Job Description

• Execution of the medication management plan including periodic review of the documentation, observation of the effectiveness of the plan, communication to the physician when changes are needed or outcomes are not appropriate
• Continuous assessment of resident needs with updates to appropriate individuals as it relates to care, services and changes in condition
• Implement measures to ensure resident wellness and safety and the appropriate documentation of those measures
• Work in a team fashion to meet resident needs
• Respond to emergencies timely and in a calm and competent manner
• Report incidents and changes in resident condition to Healthcare Director and with the appropriate documentation
• Be an active part of all activities including community events
• Be present and assist with the delivery of meals in the dining room, monitoring resident acceptance of diets and assuring adequate nutrition
• Maintain high standards with regard to record keeping, resident and staff documentation
• Establish and maintain a positive working relationship with all departments
• Identify risk factors for residents and initiate interventions including the appropriate documentation of that intervention
• Provide a thorough report of resident status to on-coming nursing team, at each change of shift, and in the Daily Log
• Provide motivation and direction to CNA/RMA's
• Must be certified or willing to take and pass the certification classes required by the state in which employed

Requirements

• Current LPN license to practice nursing in the state in which the community is located, holding a license in good standing with the Board of Nursing
• Experience working with the senior population, preferably in assisted living or LTC
• Must be a team player who works well with others and has experience supervising staff
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing
• Demonstrated ability to manage time efficiently with strong organizational skills
• Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs
• Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays
• Must attend all required trainings, CEU's and staff meetings
• Must be certified or willing to take and pass the certification class required by the state in which employed
