Qualifications

• Valid Ohio Licensed Practical Nurse (LPN) license and CPR required.
• Minimum of 2 – 4 years’ direct patient care experience in hospice or home care.
• Licensed driver with a vehicle in good condition.

Responsibilities

• Provide bedside care to hospice patients and support families.
• Work closely with the RN Case Manager.
• Ensure compliance with Medicare and Ohio Department of Health regulations.

Otterbein is seeking a PRN Hospice LPN, to assist with on call needs, continuous care and routine visits to our patients. Responsibilities

• We are currently seeking an experienced and professional Hospice LPN.
• This role is responsible for providing assistance in caring for our Hospice patients.
• You will provide bedside care to patients, support families and caregivers and work closely with the RN Case Manager.
• Otterbein Hospice complies with Hospice Medicare Conditions of Participation, and Ohio Department of Health regulatory requirements.

Qualifications

• Education: Graduate of approved nursing program
• Licensure/Certification: Valid Ohio Licensed Practical Nurse (LPN) license, CPR
• Experience: A minimum of 2 – 4 years’ direct patient care experience in hospice or home care
• Since this is a community based ministry, all LPN’s are required to be licensed drivers with automobiles in good working condition and have automobile insurance in accordance with Ohio laws.
