Job summary

Summit Health Management is seeking an LPN for a physician-led, patient-centered care team in the New Rochelle area. You will support office visits, charting, and clerical tasks while ensuring safe, high-quality patient care under supervision.

The ideal candidate holds an LPN license (Connecticut) with BLS certification and basic IV/phlebotomy skills, and is comfortable working in a fast-paced clinical setting that values patient outcomes and efficiency.

Qualifications

• Graduate of an LPN school program.
• Current licensure as LPN in state (Connecticut).
• BLS certified upon hire.
• IV and/or phlebotomy skills highly preferred.

Responsibilities

• Introduce self to patient and escort to exam room.
• Prepare patient for exams and procedures; explain treatments and physician instructions.
• Administer medications including injections and IV therapy within licensure.
• Provide basic patient care and treatments (wound care, catheters, dressings).
• Schedule follow-up appointments and communicate delays as needed.
