Job summary

Harmony Home Health and Hospice, LLC., Albuquerque, NM, is seeking a Licensed Practical Nurse (LPN) to join our team. You will create and manage individualized plans of care, perform direct patient care, and collaborate with RNs and physicians to deliver compassionate home health services.

This role emphasizes thorough documentation, patient and family education, and adherence to the State Nurse Practice Act.

Qualifications

• Graduate of an accredited practical nurse program and licensed in NM.
• Home health experience preferred but not required.
• Must have valid NM LPN license and comply with professional standards.
• Good verbal/written communication and organization skills.

Responsibilities

• Provides direct patient care as defined in the State Nurse Practice Act and home health duties.
• Creates and manages individualized plans of care for patients.
• Documents services and tracks outcomes of interventions.
• Educates patients and families on diagnosis and self-care.
• Assists RN/physician with home health tasks and team coordination.
• Maintains equipment and aseptic technique; participates in QA activities.

Job Summary

We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily create and manage a plan of care for individual patients. You are an LPN with great attention to detail and a big heart. You take pride in creating personalized care plans, observing, and communicating effectively in your patient’s best interest.

Essential Duties and Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to Home Health Care.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• Provides accurate and timely documentation of patient services to reflect the plan of care and documenting outcomes of interventions.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self‑care.
• Assists the RN and/or physician in performing specialized duties related to home health care.
• Participates in coordination of home health services, appropriately reporting the identified needs to the interdisciplinary group (examples include home health aide, OT, PT, SW, ST, dietician, or clinical supervisor).
• Uses and prepares equipment and supplies effectively, adhering to aseptic technique.
• Provides appropriate pain/symptom management and evaluates patient’s response to treatments/medications.
• Participates in personal, professional growth and development and in the organization’s quality assessment performance improvement teams and activities.
• Performs other duties as assigned by the registered nurse.

Physical Demands and Work Environment

Physical demands: The employee is required to perform job related duties that may require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, pulling, talking, hearing, and maneuvering up and down stairs. The employee must occasionally lift and/or move up to 50 pounds. Vision requirements include close vision, distance vision, color vision, peripheral vision, depth perception, hand‑eye coordination, and the ability to adjust focus.

Work environment: While performing duties, the employee is exposed to communicable diseases and weather conditions. The noise level is usually minimal.

Qualifications and Experience

• Graduate of an accredited practical nurse or vocational nursing program (LPN/LVN). (Diploma or transcripts to be provided to agency.)
• Prior home health experience preferred but not necessary.
• Currently licensed as an LPN in the State.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication, and organization skills.
• Ability to exercise sound judgment.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Current TB test, CPR card, auto insurance, and driver’s license must be kept on file.
• A criminal background check must be performed before hire.
• Complete and pass skills evaluation.
