Job summary

Brooks Rehabilitation in Jacksonville, FL is seeking a Licensed Practical Nurse to join our inpatient care team. You will administer medications, perform basic nursing tasks under RN direction, and participate in care planning for rehab patients on 12-hour shifts.

Qualified applicants hold FL LPN licensure, at least 1 year in acute or post-acute care, IV certification, and current BLS. The role includes weekend coverage, holiday rotation, and a comprehensive benefits package.

Responsibilities

• Documents, observations and care are provided per policy and procedure.
• Administers medications and basic nursing care measures.
• Assists in caring for rehab patients across age groups and developmental stages.
• Admits, transfers and discharges patients as needed.
• Observes, records and reports subjective patient symptoms.
• Takes and records vital signs and monitors intake/output.

Offering a comprehensive system of care, Brooks provides inpatient and outpatient services, skilled nursing, assisted living, and memory care and impacts lives beyond clinical settings through community programs designed to enhance the quality of life for individuals with physical disabilities.A Licensed Practical nurse performs nursing functions under the direction of a registered nurse, follows the plans of care, participates in care planning, and performs nursing actions in accord with company policy and state law.Key Responsibilities:Documents, observations and care are given in accordance with company process and company policies and procedures.Administers medications.Administers basic nursing care measures to the rehab patient and assists in giving care to the acutely ill rehab patient relative to patient age, developmental needs, and understanding.Admits, transfers, and discharges patients.
