Job summary

Avamere Rehab of Junction City is hiring a Licensed Practical Nurse to provide essential nursing care under RN supervision. Responsibilities include vital signs, medication administration, wound care, and assisting residents with daily activities to support comfort and safety.

You will document observations, monitor patient status, and collaborate with CNAs and the nursing team. A current LPN license, CPR/BLS, and ELN experience are preferred for this role.

Qualifications

  • Must possess a nursing degree from an accredited college or university.
  • Must hold an active LPN license in the state.
  • CPR/BLS certification required during employment.
  • Experience in long-term care or geriatric rehab is preferred.
  • Experience documenting in electronic medical records.

Responsibilities

  • Provide basic nursing care under RN supervision.
  • Accurately measure and record vital signs.
  • Administer medications per physician orders and facility protocols.
  • Perform wound care and monitor for infection signs.
  • Assist with hygiene, dressing, bathing, and mobility.
  • Support nutrition and hydration and assist with feeding if needed.
  • Document care provided, meds, and observations.
  • Communicate changes in patient condition to the RN and care team.

Licensed Practical Nurse

Status: NOCs (6p-6a)

Schedule: Thursday thought Saturday with every other Wednesday.

Location: Avamere Rehab of Junction City - 530 Birch Street, Junction City, OR 97448

Job Summary

Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)

support and comfort to residents and their families.

Essential Duties and Job Responsibilities
  • Basic Nursing Care:
  • Vital Signs:Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.
  • Medication Administration:Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.
  • Wound Care:Perform wound care, including changing dressings and monitoring for signs of infection.
  • Hygiene and Mobility:Assist residents with personal hygiene, dressing, bathing, and mobility.
  • Nutrition and Hydration:Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.

Patient Monitoring:

  • Patient History:Record a patient's history and current condition.
  • Observation:Observe and document changes in a resident's condition, reporting any concerns to the RN.
  • Communication:Communicate effectively with residents, families, and other healthcare professionals.

Collaboration and Supervision:

  • RN Supervision:Work under the supervision of an RN, following their guidance and instructions.
  • CNA Supervision:Supervise certified nursing assistants (CNAs) in providing basic nursing care.
  • Teamwork:Collaborate with other healthcare professionals to ensure residents receive comprehensive care.

Other Duties:

  • Patient Safety:Maintain a safe environment for residents, following infection control protocols and other safety guidelines.
  • Documentation:Accurately document all care provided, including medications administered, vital signs, and any observations.
  • Emotional Support:Provide emotional support and comfort to residents and their families.
  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an LPN in this state.
  • Must maintain and have an active CPR/BLS during employment.
  • Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
  • Experience in electronic medical records documentation.
