Job summary

Eliot Community Human Services is seeking a dedicated Respite LPN/RN in Boston to oversee clinical care for adults with mental illness within a short-term restorative program. You will guide clients, provide direct medical care as ordered, and build supportive relationships in a compassionate, community-focused setting.

Position schedule is Monday–Friday, 9am–5pm, with a $5,000 sign-on bonus. The role emphasizes crisis management, communication, and problem-solving to support clients and staff.

Qualifications

• Licensed LPN or RN in Massachusetts.
• Experience working with adults with severe mental illness.
• Valid Massachusetts driver's license.

Responsibilities

• Provide education and consultation to clients and staff on medical health, medications, and general medical topics.
• Deliver one-on-one training tailored to individual needs.
• Develop and lead groups and trainings on health and wellness topics, including nutrition, exercise, and smoking cessation.
• Assist clients in accessing community resources that support health and wellness goals.
• Meet with clients and staff to discuss diagnoses, causes, symptoms, and treatment options.
• Perform direct medical care as ordered by physician, including vitals or wound care.
• Review medical documentation and coordinate care as needed.
• Coordinate overall medical care and ensure follow-up.
• Provide health education and skill-building interventions.
• Support team by introducing tools for symptom assessment and response planning.

Eliot is seeking a dedicated Respite LPN/RN to join our team. Eliot’s Respite program is a short term restorative service that assists adults with mental illness in obtaining and maintaining community living. As a Respite LPN, you will be responsible for providing oversight, guidance and assistance to clients in understanding their medical conditions; as well as performing direct medical care that is needed. Our ideal candidate will have the skills to establish supportive relationships, excellent communication, crisis management and problem solving skills.

Responsibilities

• Provide education and consultation to clients and staff on medical health, medications, and general medical topics
• Deliver one-on-one training tailored to specific individual needs
• Develop and lead groups and trainings for clients and staff on general health and wellness topics, including nutrition, exercise, and smoking cessation
• Assist clients in accessing community resources that support their health and wellness goals
• Meet with clients and staff to discuss medical diagnoses, causes, symptoms, complications, and available treatment options
• Perform direct medical care for a Person as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc. This includes providing interventions specific to Person’s IAP (Individual action plan) for medical/health and wellness goal areas.
• Review medical documentation for each assigned client and provide consultation and coordination of care as needed
• Coordinate overall medical care for clients and ensure necessary follow-up is completed
• Provide health and wellness interventions focused on education and skill-building
• Support the team and clients by introducing tools for ongoing symptom assessment, evaluating treatment effectiveness, and identifying appropriate responses or next steps

Qualifications

Annual Salary: $60,000 USD - $72,560 USD
