Qualifications

• Candidates must have a current LPN or RN license in the State.
• Minimum one year of experience in a Long-Term Care setting preferred.
• Knowledge of Matrixcare preferred.

Responsibilities

• Provide direct and individualized nursing care to patients.
• Consult and coordinate with healthcare team members.
• Administer medications and record patients' medical information.
• Monitor and adjust specialized equipment.
• Change dressings and prepare equipment for physicians.

LPN/RN Responsibilities

• Provide direct and individualized nursing care to patients based on the application of scientific nursing principles.
• Consult and coordinate with healthcare team members to assess, plan, implement and evaluate patient care plans.
• Prepare and administer medications orally, subcutaneously, or through an IV, and record prescribed medications. Report adverse reactions to medications or treatments in accordance with policy regarding the administration of medications by a licensed RN.
• Record patients’ medical information and vital signs.
• Monitor and adjust specialized equipment used on patients, and interpret and record electronic displays.
• Initiate corrective action whenever a patient displays adverse symptomatology.
• Provide bedside care for a wide variety of medical patients.
• Change dressings, insert catheters, start IVs.
• Prepare equipment and aid physicians during examination and treatment of patients.
• Respond to life‑saving situations based upon nursing standards and protocol.
• Participate in discharge planning.
• Utilize time‑management skills.
• Record all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
• Perform other position‑related duties as assigned, depending on assignment setting.

LPN/RN Requirements

• Current LPN or RN license in State.
• At least one year of experience as a LPN or RN in a Long‑Term Care setting preferred.
• Knowledge of Matrixcare preferred.
